MDL profylaxe bij ingrepen*

General comments

Ingreep Antibiotica
ERCP

Alleen indien infectie (galwegobstructie) of verdenking onvolledige drainage of PSC: 

Ceftriaxon 2 gram iv, zie ook cholangitis 
PEG-plaatsing  Cefazoline 1 gram iv eenmalig 
PEC (percutane endoscopische colostomie) - plaatsing (Rijnstate) Amoxicilline-clavulaanzuur 1200 mg iv 4 dd op de dag van ingreep, gevolgd door amoxicilline-clavulaanzuur 3dd 500/125 mg po gedurende 4 dagen 
Zenker's divertikel  Cefazoline 1 gram iv en metronidazol 500mg iv eenmalig
EUS-geleide punctie en evt. drainage cyste 

Indien alleen diagnostische punctie: geen antibiotica

Indien drainage: dan behandeling met pipperacilline-tazobactam 4 dd 4,5 gram of  oplaaddosering 1 dd 4500mg gevolgd door 18000 mg/24 uur continu iv, 

na ingreep, op geleide van kweek aanpassen

Punctie transrectaal:

  • clipplaatsing
 Cefazoline 1 gram iv en metronidazol 500mg iv eenmalig 
EUS-geleide punctie/drainage transrectaal bij abces  Behandeling: Ceftriaxon 1dd  2 gram iv en metronidazol 3 dd 500mg iv, op geleide van kweek aanpassen
Banding bij varices bloeding Ceftriaxon 1 dd 2 gram iv, tot maximaal 7 dagen*
Banding bij oesophagus varices Electief: geen
Punctie cysteuze lesie mediastinaal Cefazoline 1 gram iv en metronidazol 500mg iv eenmalig

Alle MDL ingrepen zijn <1 uur waarbij Cefazoline 1 gram voldoende is.
