General comments
Zo mogelijk tevens chirurgisch debridement.
Behandelingsduur 6 weken-6 maanden op geleide van de resolutie van infectie.
Bij aangetoonde gevoeligheid eventueel step-down naar posaconazol of isavuconazol tabletten onder TDM.
Posaconazol
iv: eerste dag 2 dd 300 mg, daarna 300 mg 1 dd
po: Bij voorkeur tabletten: eerste dag 2 dd 300 mg, daarna 300 mg 1 dd po. Eventueel suspensie indien tabletten onmogelijk: 400 mg 2 dd po (met voedsel) of 200 mg 4 dd (zonder voedsel).
Posaconazol intraveneus is niet standaard op voorraad in de apotheek in Rijnstate, Slingeland, Gelderse Vallei en Rivierenland.
Voor het voorschrijven contact opnemen met de arts-microbioloog én de ziekenhuisapotheker.
