General comments
profylaxe éénmalig; peroperatief te herhalen wanneer ingreep > 4 uur duurt (c.q. langer dan 2 x de halfwaardetijd van het betreffende antibioticum) en/of bij > 1,5 L bloedverlies, dit geldt niet voor vancomycine.
Vancomycine moet minimaal in 1 uur inlopen.
