Creatinineklaring kleiner dan 30 ml/min in geval van PJP:

Behandeling van Pneumocystis Jirovecii pneumonie: eerste dag de standaarddosering van 1920 mg 3x per dag, vervolgens onderhoudsdosering 1920 mg 2x per dag, op geleide van de plasmaconcentratie.

Profylaxe van Pneumocystis jirovecii pneumonie: eerste dag de standaarddosering van 960 mg 1x per dag, vervolgens onderhoudsdosering 480 mg 1x per dag. Overweeg monitoring van de N4-acetyl metaboliet.
