Creatinineklaring kleiner dan 30 ml/min of nierfunctievervangende therapie in geval van PJP:

Behandeling van Pneumocystis Jirovecii pneumonie: eerste dag de standaarddosering van 1920 mg 3x per dag, vervolgens onderhoudsdosering 1920 mg 2x per dag. Monitor de spiegel van trimethoprim, sulfamethoxazol en N4-acetylsulfamethoxazolmetaboliet.

Profylaxe van Pneumocystis jirovecii pneumonie: 480 mg 1x per dag.

Renal elimination: 20%

Renal function-replacement therapy

CAPD: Eerste dag de standaarddosering van 960 mg 2x per dag, vervolgens onderhoudsdosering 960 mg 1x per dag. Bij PJP: zie boven.

Hemo dialysis: Eerste dag de standaarddosering van 960 mg 2x per dag, vervolgens onderhoudsdosering 960 mg 1x per dag. Bij PJP: zie boven.
